Where can I document evaluation reports?

Where can I document an evaluation and how is it linked to a particular patient?  When I open a "practice" patient I see no way to do this.

I put my own template in the "progress" section, but see no way that this form is to be put into a patient's file.  Can you help?

(I am a speech pathologist.)

  I'm an SLP, also. We upload our evaluations into the UPLOAD FILES on the righthand side of a client's main page.

  We're working on keeping track of re-evaluation dates right now. So far, we are doing this by putting Administrative Notes in with the re-evaluation date.

  We need to be able to bill for evaluation reports that the therapist do for the clients.  We have a CPT code, but how do I have it show up in the billing without  making it an appointment,   If I make it an appointment then the client gets a reminder of the appointment and it is confusing for the clients.
