I submitted a claim yesterday and saw this morning that it got rejected. I have been seeing this client for months and none of my other claims have been rejected. None of my information or the client's information has changed. I looked over the insurance claim and it appears that all the information is correct. The rejection message was: NEED BILLING PROV STREET ADDRESS (21)

Has anyone been able to get a resolution to this issue? We suddenly have a clients claims constantly rejected for Meritain and we receive the same error regarding Practice Address. We use a PO box for all of our other claims with no issues and we have other Meritain clients that are processing correctly. We also had about 4 or 5 claims process correctly and now we cannot get the clearinghouse to accept these claims.

There are many reasons why claims may get rejected, which is why we need to investigate each case separately. To get in touch with them, please submit a Help Request. I received 'rejected claim' for the same person for 2 different services dates. I have verified with the patient and the insurance that the information is correct.Rejected due to the following error: ACK,RETURND AS UNPRCESSBLE CLM- CLM,ENCNTER BEEN REJECTD AND NOT ENTERD INTO ADJUDICATION SYS.:SUBSCRIBER AND SUBSCRIBER ID NOT FOUND. Why has this claim being rejected. I spoke with representative from Blue Cross Blue Shield she referred me to you
Was the claim rejected with the error NEED BILLING PROV STREET ADDRESS (21)? What we found was there was an issue with our PO Box being listed on the claim form. We had to contact support with the URL for the client and claim form in question and they corrected the issue on their side. Every so often we continue to see this error again.
