    I received 'rejected claim' for the same person for 2 different services dates. I have verified with the patient and the insurance that the information is correct.

    Rejected due to the following error: ACK,RETURND AS UNPRCESSBLE CLM- CLM,ENCNTER BEEN REJECTD AND NOT ENTERD INTO ADJUDICATION SYS.:SUBSCRIBER AND SUBSCRIBER ID NOT FOUND. (21).

    I had the same experience yesterday 5-4-20.  No changes in any biller or recipient address data.  I put a help request for this one.   Did you resolve this?  It occurred across three different insurance providers.

    Has anyone been able to get a resolution to this issue? We suddenly have a clients claims constantly rejected for Meritain and we receive the same error regarding Practice Address. We use a PO box for all of our other claims with no issues and we have other Meritain clients that are processing correctly. We also had about 4 or 5 claims process correctly and now we cannot get the clearinghouse to accept these claims.

    Hi Micha - I think it would be best if a member of our Insurance Group took a look at these claims for you. There are many reasons why claims may get rejected, which is why we need to investigate each case separately. To get in touch with them, please submit a Help Request.
