Beacon Health Issues

I was told by Beacon Health systems that I needed to request an intermediatory authorization enrollment form from Simple Practice to be able to submit to them under my new address and tax ID. Has anyone else had this issue? Or has any helpful suggestions for getting everything set-up with Beacon. 

Also in SP, they give you a for to follow to submit claims, but according to Beacon when you call them, this process and code you’re supposed to submit on SP to be able to send claims electronically doesn’t exist any longer. Anyone else having this experience?
