Does anyone have a template of a Clinical Supervision contact log they would like to share? I need a contact form that I may use for my supervisees to sign weekly. Is it possible for both me & the supervisee to electronically sign? Any suggestions?

  We created a supervision note in the assessment section of the EHR and each of our interns and supervised practitioners have a client (employee) chart in the system which we use specifically for that purpose. We also use the same assessment form for clients that we have supervision on so we have record of what was discussed for each client.
