PHQ-SADS or PHQ-15 Template Addition request

Hi Folks,  I made the following request to Simple Practice and they suggested I post the request here for your comments. 

"I'd like to request that SP include the PHQ-SADs in the Template Library. PHQ-SADS includes the PHQ-15 (somatic symptoms), GAD-7, and PHQ-9 in one document. This form is very useful for a large portion of clients who have a variety of somatic complaints, often in connection with their anxiety, depression, or trauma history. Having all 3 questionnaires in one seems less intimidating than asking clients to fill out 3 separate questionnaires, in addition to all the other intake forms that are required. 

Short of including the PHQ-SADs, I would ask that SP at least add the PHQ-15 questionnaire, which would allow us to send all 3 of these questionnaires separately to clients."

    I, too, would greatly appreciate the PHQ-9 and GAD-7 be available in the SP template library.  How are others navigating using these now via telehealth with the SP client portal?
