Can someone please help me understand the difference between these two questions in the "Treatment Plan & Goals Note"?

1. What actions has client taken to meet their long-term goals? 

2. Which plans were carried out by client to meet their long-term goals?

    Hi Jacilyn - The difference between the two questions seems to be: Question two is asking about the specific set plans clients followed through with to meet their long-term goals; while question one is asking about any kind of action that was taken that fell in line with meeting long term goals. It's a minor distinction, if you'd like to delete one of these questions, you can do so by clicking on the note template in Settings > Notes & Forms.

To give you a little context, the Notes and Forms Template Exchange is primarily used for clinicians to share their ideas for new templates, or share copies of custom templates they've created. We monitor it from time to time, but in the future, I'd recommend posting your questions about the platform, or pre-populated templates, in the General Q&A Topic Board.
