Create a template that contains a 5 column x 20 row Table.

Hello, is there any way I could make a template that is a Table? For example, I would like my template to include a Table that has 5 columns and about 20 rows.

I might use this chart when I am helping a client to assess the pros and cons of a difficult choice.

I might use this chart when I am helping a client to identify the qualities they value in a good friend, and then to assess which of their friends have most of the qualities.

  Hi Penny, while there isn't currently a way to do this with SimplePractice's Note and Form Template Builder, thank you for your suggestion.   I was looking for this in the help menu too. I want to create a fillable medication chart to send to patients during the intake process.
