Emergency contact and other parent info

When creating a new child client, I need information on both parents.  Is there a way for parent #1 to enter in parent #2 and the emergency contact's information? Or do I need to email portal info to parent #2 and the emergency contact separately? 

How do people get a dad's phone number if the mom was the one who filled out the forms?  Do people create a new form with phone numbers for the parents to complete?  Are people on simple practice just not getting emergency contact info?

    Hi Susan,

    Our Demographic Information form requests emergency contact information.

    When adding two parents to a minor's profile, each parent's Demographic Information form must be sent to that parent's email address in order for the information to auto-populate into that parent's profile in your account.

    If parent #1 has access to parent #2's email, then parent #1 could complete parent #2's forms, but each person's paperwork must be completed from that person's own Client Portal.

    If someone does not enter the emergency contact initially, does this then have to happen by creating their contact manually, and then connecting it to them...they cannot go back and enter the info themselves thru the client portal?

    Hi Elise, if the client has already completed the Demographics Information Form, you can resend it to them in order for them to fill out information they missed the first time. The information the previously entered will auto-populate in the form, so they won't have to reenter any information. If they haven't completed the form, it should still be in their Documents tab in their Client Portal.
