Referral Template with client name
AnsweredIs there an opportunity to have a form to send back to the referral source as an update on appt scheduled or on missed appointments? This form should include patient-specific identifiers that clinicians can choose.

Hi Lisa, at this time, there's no way for you to communicate with an outside provider through SimplePractice. You also cannot create custom forms through SimplePractice. However, you can go to a client's file and click on the Appointment Status Report link in the BILLING OVERVIEW box to the top right. You can adjust the date range for this report to get a list of all the appointments you've scheduled for this client, the CPT Codes for the appointments that were not cancelled. You can then go to Reports > Attendance Report to see how many appointments each of your clients have attended, "Cancelled," "No-Showed" and "Late Cancelled." If there are specific data points you'd like to include in a report, let me know so I might point you to a specific report we have in the system!
