    Hi Tina, some customers have clients sign into their Client Portal on the clinician's device during the session. Others change the template type from Intake Form to Assessment in Settings > Notes & Forms. Click on the Standard Intake Questionnaire form in question, use the dropdown menu at the top to change the template type to Assessment. Next, go to your client's Overview page > New > Assessment and use the dropdown menu to select Standard Intake Questionnaire as the Assessment Template to fill it out on behalf of your client. If you fill out an Assessment for your client, they won't be able to add their electronic signature to the form, but you can print it out and have your client sign it by hand, then scan it back into their client file as an Uploaded Document. Click the Upload button to the top right to reupload the signed form. Let me know if you have any other questions!

    Ruth - Thank you so much for your fast response and information for a helpful work around! Very much appreciated. While doing as you suggested, I also noticed I can move questions up and down as well as add new questions. This makes it much more usable for me. :)
