Deleted Several Original Consent Forms

Forms Like: Clinician Info Share I assumed these deleted forms would remain in template library, but it is not there. Is there a way to add all the deleted forms and keep the customized ones? I was ignorant and assumed I could pull them back in easily. Thanks!

  Hi Nicole, you can re-add forms you deleted from your Settings > Notes and Forms > My Notes & Forms tab, if they were originally in the Template Library. However, you can't recover any custom templates you created, or any customizations you made to a template you found in the Template Library. If by Clinician Info Share, you're referring to the Release of Information Form, then you can go to Settings > Notes and Forms > Template Library and hit the +Add button to the right of the ROI (Release of Information Form).

  I'm having the same issue.  But the proposed solution does not work.  I am trying to recover a SP template that I deleted (not knowing it would disappear from the library).  However, when you go back to the library, you can't simply click the +add button because the form isn't there!  Please advise SP.

  Hi Melissa, it looks like you hadn't upgraded your account to the Professional Plan after your trial expired, but have since done so. You should now have access to all of our templates in Settings > Notes & Forms > Template Library!
