Anyone have a sample of a family intake form stat?

Hi,

I am hoping not to design my own family intake form and am wondering if anyone has one that I could use?  Or....has anyone modified a current form that can be signed, etc...as a family intake form?  Any suggestions.

  Hi Susan, we have a Couples Initial Intake form that's available in Settings > Notes & Forms > Template Library, but we don't have a Family Initial Intake form in our Template Library. You can edit the Couples Initial Intake form, or the one of the individual intake forms in your Template Library to fit your practice needs.
