    Hi Nancy, when you're creating, or editing an Intake Form in Settings > Notes & Forms, you can select the Multiple Choice (Checkboxes) answer type!

    Hi. My need is slightly different:  I need to create a table with 10 rows. Each row needs the following four (4) sections:

    Issue/ Level of Interest/ Techniques to Use/ Date Used

    And I need to complete each section with my own written answers.

    (This is for my use as a treatment plan. Not for clients.)

    How can I make a note/form for this? Your help is appreciated. Thanks!
