Can I use my own Intake Forms (consent, HIPPA, release, etc) - Can clients sign my forms electronically?
AnsweredCan I set in your system my own Intake Forms? Would clients be able to sign my version of Intake Package electronically in their portal?
I have been using a set of forms for the Intake of new clients, can I continue to use them in your system? How do I set them up in the system? After including my form version in the system can clients sign them electronically in their portal?
Can I use a combination of your intake forms and mine?

Hi Miriam, you can add your own forms and documents as read-only PDF files for your clients to download and sign outside of SimplePractice. However, if you want your clients to sign your forms and documents electronically through SimplePractice, then you'll want to recreate them through our template builders. To upload your forms, go to Settings > Client Portal > Shared Documents and Files and click +Downloadable File under the Files (Read-Only) section. To edit, or create new Consent Documents, go to Settings > Client Portal > Shared Documents and Files and hit Edit next to the default documents, or hit the +Consent Document to create a new one. You can copy and paste text from your Consent Documents to these text fields. Next, go to Settings > Notes & Forms and click on any Intake Form listed to edit it, click +Create New Template to start from scratch, or toggle over to the Template Library tab at the top to find another form that you maybe able to work off of. Once you've clicked the + icon next to a form in the Template Library, you can click on it in the My Notes & Forms tab to edit it.
