Release of Information form not showing my name

    I had my intake  forms all nicely filled out with my practice info at the top.

    Then I changed my user name and password to sign in,

    and now my clients receive forms with headers like [clinician''s name] [practice address] etc.

    I can't edit them even though I've watched the webinar.

    What can I do?

    Hi Maggie, it sounds like the changes to your Consent Forms that you made may not have saved. You can find your Consent Forms under Settings > Client Portal > Shared Documents and Files and click on the name of the Consent Form to update it. If you're still having trouble with this feature, please let me know - I'm here to help.

    I'm having the same problem, and cannot find "Consent Form" anywhere in the Notes and Form section.  What am I doing wrong?  I need to send intake documents to a new client and cannot figure this out!
