    Hi Ana, you can input some of the fields you're referring to in the client's file > Edit > Billing and Insurance, under the Insurance Claim/ CMS Fields section. Once you've scrolled down to this section, check the box to "Show additional claim fields" and fill out the information as needed. Here's a screenshot:
