Informed Consent/Release for 2 Clinicians in a Single Practice Sharing 1 Client Record

Hello!  Do any of you fellow group practice owners have a form that clients sign allowing two clinicians to share a single client record (and see each other's notes)? I have several situations where a client is working with one provider for individual therapy then starts either couples or family therapy with another provider.  We have used strategic workarounds up until now but I'm wondering if someone has created a release they'd be willing to share.
