    I have a version of the intake paperwork as a progress note called clinical mental health intake interview. I would just copy your intake paperwork a progress note and proceed as if it were the note. You could also do this as an “assessment” if you still wanted to do your standard progress note for that session as well.

    I just checked. I think as long as it’s not your default intake, so it’s a copy, you can go in and change it to an assessment or progress note. This is much easier than trying to print fill out scan and upload.

    I would like to learn how to do this as well. I have forms that I would like my patients to complete each session, like a neck pain index. Some patients can only respond while in the office/treatment. I would love to be know how to "open" and complete a form (or specific assesment in my library) in real-time and have it append to that day's treatment notes for that patient.

    I would also like to learn how to do this as well. Some of my clients don't complete the standard intake and I'd like to help them complete it in session and then have it go in their chart.

    Any update with this? I want to be able to add to the intake questionnaire and do screenings with client in session.  I figured out how to change it to "assessment" in the notes and forms section, but then how do I pull it up when with a client and save it into client's chart?

    I would like to be able to help older clients that do not know how to use a computer to fill out intake questionnaires and screening and save it to their chart.  This thread has been going for 2 years with multiple request. I hope we can get answers soon.
