Minor and Adult Signature

How do I create two or three separate signatures in one document?  In Illinois, minors 12-17 need to sign the consent forms, as well as the adult, and a witness.  I am able to add three signature spots but when it is completed, it just shows as the adult's signature.

  I believe you can edit the form. When you edit it, you will have to change the category to intake forms, so that you can add another signature. I hope you will figure it out. I'm not tech savvy but somehow i found a way.
