Has Simple Practice stopped offering customer support?

I'm finding it impossible to locate and create forms I  need utilizing the help section directions. Has Simple Practice eliminated contact with a real person from customer support?

  Hi Miriam,

  To locate and create forms, go to Settings > Notes and Forms. On this page you can view all forms that you have previously created and create a new form by clicking the +Create New Template button at the top. Additionally, our process of contacting SimplePractice via email, chat, and phone support have not changed.

  I began a Diagnostic Assessment this morning and didn't completely finish it. I saved it and now it is gone. How do I get it back? How do I talk to someone simple practice on the phone for phone support?
