??What to do with Neuropsychological and Psychological Testing Documents??

Does anyone keep paperless files for testing cases?  Do you upload them to the client's file (without sharing them) and then shred them?

  I am a Psychologist just added to a new practice. Is there somewhere on simple practice to find or make templates for a Comprehensive Psychological Evaluation, Forensic Evaluation, Etc.?
