Medi-Cal as secondary

If someone has medicaid FFS plan as secondary (such as Medi-Cal), do I collect the copayment from their primary commercial plan before billing secondary? Are they off the hook for their copay because they have Medi-Cal? I am weary of charging someone with Medi-Cal, because I know balance billing and no-show fees are not allowed.
