Well... I was trying to upload a template for other to use if they wanted to but I don't see how.  The title "Note and Form Template Exchange" seems a bit misleading unless I'm missing something.

I work with couples and specifically "Mixed Orientation Relationships".  Typically it's when one spouse comes out or is discovered as gay, lesbian, or bisexual.  This was a Form for the non-straight spouse.  I thought some might find it useful. 

Oh well.  Hopefully I'm just missing something and I'll learn how to do it, if there is a way.
