It is not in the Template Library under Assessments as Simple Practice articles have indicated. Please advise where it is located? Thank you.

  Hi Mary,

  This template has been added to every customers account in the Settings > Notes and Forms > Template Library > Assessments section of the account. If you are unable to find it, I recommend creating a Help Request so that our Customer Success Team can investigate. Click on the ? icon in your account > Ask Questions > No > Contact Support > Send an Email Request.
