Where are others adding this into their paperwork to send out?

trying to decide best place to do so.

  I'm fine with disclosing the financial obligation but to pre-diagnose someone and estimate the frequency of treatment is unethical to me so I'm sending them after the first session. I have my first new self-pay today and I'm struggling with the template.
