When Sending out the generic Massage Therapy Intake form

When sending a new client a form there is a field that requires a client to say 

one of 4 options or they can not finish the form.

and some non of the options even apply to them.

how can we also ad N/A to that portion of intake?

  Hi Krystalyn,

  In order to resolve this, you will either need to add N/A as an answer option in this form, or make the answer not required. To do this, go to Settings > Notes and Forms > Edit next to the form > edit the answer with one of the two options > Save.
