The provider can create a Good Faith assessment for the individual by clicking on the blue "new" button. The form can only be completed if you open another window with the client's information next to your "assessment" because it doesn't population by itself. Once created you can save it as a PDF and subsequently upload it as a new document in the client's record. However, you can not send it through the portal - unless someone knows how to add it to the list of documents because "assessments" are not shared in the portal. Consents and policies can be listed and shared with the client. It seems that the PDF version can be emailed to the client but since it has PHI I don't want to send it through email.
