AnsweredHello, I'd like to send a release of information to a client. And, while I figured out how to edit it from the template. I don't see a way to send it. Someone once said that when they entered the client portal it was not there. I think you have to manually send it. But then how do they send it back to you?

I have the same question as Maria. I need to be able to fill out the agency information before I send it to the client. How do we edit, then send a release customized to the client. This isn't answered anywhere else so I feel like it's just not possible the way they have it set up. This should be a lay-up, not something I'm searching for answers online and finding none.

Whether providers fill out a ROI before sharing with/sending to client, or just send/share our own blank ROI, can the template be edited so that the client can COMPLETE ONLINE / electronically, w/o either of us having to PRINT the ROI? Can we make other templates/forms that clients can complete on their computer or smart phone? This would be SO helpful.

To share a form with a client, go to the client's Overview page > Share > select form > Review and Send. There is also a MAJOR problem of not having the ability to rescind consent -either delete it and risk a lawsuit or leave it and risk a lawsuit-this is what needs to be fixed a way to rescind to include date of same and comment to be made by therapist
As of 6/20/24 clt rescinded consent. Per clt no further information will be shared for example

I found in another thread the solution for this.
Essentially, you add "other document" from the overview page, and one of the options that will come up is a ROI. You can fill out the ROI, sign it and then share it to your client and have them sign it. Clearer instructions are here.

I'm not sure what you mean. The ROI that shows up starts with "I authorize [NAME OF PRACTICE or CLINICIAN'S NAME] to:" and at the end after you saved it, you need to sign it so that you are listed as the "provider" at the bottom. Then, you can click "share" and send it to your client to have them sign it. I just did this and I have both of our signatures designating me as the one being released to or release to gather info.

I tried to add the directions to add ROI to template library in order to share as "other document". However, there are 2 versions in the template library - one titled "ROI" which, when previewed, is titled "Release of Information Consent", and another called "ROI Short" which, when previewed, is titled "Authorization for Disclosure of Information". I checked both options in my template library so they would show up in "other", however only the ROI shows up, which is not the form I want. Even unchecking that from my library did not allow the simpler, ROI short version to be selected in "other" documents.
I've tried doing the workaround someone else suggested and customizing to client, then deleting, but it is a hassle. I hope they can simplify this process!
