Does Simple Practice provide a service for a fee to customize a form for an account holder if given a hard copy? If someone else customizes a form for you outside of your account, can it be uploaded into one's individual account?

Is there anyone in the community that is good with customizing forms and if so, what's your fee?

  Hi Patricia,

  You can customize forms within SimplePractice by clicking on the form in the Settings > Notes and Forms section to make any edits you need. Additionally, you cannot upload a form that is fillable by clients.
