New tiered insurance filing? Keep track? How is it billed?

I used t have an insurance tiered filing package which is gone now. For the billing is it now the first day to the last day of the month to record how many you have used, or does it going billing cycle day to billing cycle day? Answers for planning would be helpful as it was so much easier with the smaller tiered plans to knw how many had been submitted as this tracked but now isnt- thx
