I submitted the claim, the same as I have many times in the past and it is getting rejected.  The following is the message I receive...

Claim rejected

Acknowledgement/Rejected for Invalid Information - The claim/encounter has invalid information as specified in the Status details and has been rejected.
ACK,REJCTD FOR INVAL INFO-CLAIM,ENCNTR HAS INVAL INFO AS SPCFIED IN STATUS DTAILS AND BEEN REJCTD.:MISSING OR INVALID INFORMATION (A7 - 21)

I have read the other comments and looked over the claim for missing info and can not find the error.  I changed something on the original rejection, but it was rejected again. Please help.
