    Hi,

    I have submitted a number of requests to SP to allow me to work with their background program to create such a Template and post it here.  As of yet I continue to get the same cut and paste answers from the Help Desk, which refer me to these pages.  Very, very, very frustrating.  I have  Medicaid and private insurance complaint forms that I have created  and would like to generate in a user friendly format on this platform.  Absolutely no response whatsoever.  Crickets.

    I came here looking for how to load a note template with check boxes. It's disappointing to see it's not an option yet. SP, we'd really love for this to be available!

    Would love the same! Finding progress notes to be quite difficult. Especially if Simple Practice can create one that would satisfy most insurance requirements

    I found a video showing a checkbox progress note in SP here https://www.youtube.com/watch?v=pCSuyYPxOcs which looks like it was probably made by the provider from scratch. There is a document shared in the video's comments showing an example of questions/topics.
