"Backend responded with an error"

I sent intake paperwork to a new client via the client portal she reported that she received a message saying:

DOCUMENT COULD NOT BE SUBMITTED: The backend responded with an error

I delete and resent the documents to her and she received the same message. Is this an issue with Simple Practice or her operating system?
