Are Psychotherapy Notes in SP protected or discoverable?

Can Psychotherapy Notes be requested by legal entities or insurance companies ?  I have just started using SP and use Progress Notes/SOAP, but have started to use the Psychology notes section for my own reflections, intervention thoughts, etc.  However, are these notes protected?
