Child Intake Questionnaire

I have been researching the forum to see if I can find this answer but it doesn't look hopeful. Any chance SP finally got around to helping out their members and making a child questionnaire we don't a) have to create ourselves and b) have to edit an already vaguely similar questionnaire?

  That's what I was afraid of. I find it interesting there's tons of adult intakes but nothing for kids/teens. Sure, lets modify the ABA or PT one to suit our needs, but why not just create your own intake from scratch if you're going to do that? SP ought to be reaching all populations with questionnaires, assessments, and intakes so that we're able to do our jobs to the best of our ability.

  I'm having same issues. Tried to create my own and now the template isn't saving. SP any thoughts if they'll be more kid specific templates added?

  I AGREE! There used to be several intake and other forms that were child friendly, but now there isn't?

  There are also few to none child screeners. Has anyone asked about that? Or do you have to go in and recreate your own (if it isn't copyright)? I'm all telehealth so trying to figure out how to do e-screeners for children.

  Is there an aba assessment? is there a behavioral plan sample for autism and  Functional Behavioral Assessment forms
