uploading forms that can be filled out in SP

I am working with an external provider and they require me to fill out their custom forms. how can I upload them in a way that I can fill them out on SP? Please advise as it would be a big hassle to have to create all new forms.

  Hi Ana,

  Currently it is not possible to upload a fillable form in SimplePractice. In order to create a new form, you will either need to create this from scratch or use a template from our Template Library.
