I cannot find a child intake form and I know I'm not the only provider to communicate this.   Please direct to it instead of just saying you have one.

  Yes, I need an intake form specifically for children as well (in my case for mental health therapy).  I only see ABA, Speech Therapy, and Occupational Therapy intake forms for children. How can we access one for mental health therapy?  Thank you.

  You do have the ability to edit forms.  I simply took an existing intake form and made some adjustments so it met my requirements for a child intake form. Hope this helps!
