I created my own biopsychosocial form for intake assessments. How do I add my signature to this form and lock it?

  Hi Karen,

  When you fill out an assessment for a client, a blue button that says Save will be in the bottom right corner. After you complete the assessment and have clicked Save, you will see the option to click Sign in the upper right hand corner.
