    Hi Traci,

    Forms can be resent once the previous ones have been received and completed by the client. I recommend advising your clinician to create a Help Request so that our Customer Success Team can look into this and assist them. Click on the ? icon in your account > Ask Questions > No > Contact Support > Send an Email Request.
