BCBS Claims being rejected suddenly
AnsweredI've been submitting claims for our clients for well over a year without issues and all of the sudden the claims for BCBS are being rejected by simple practice with a message that doesn't make sense: "Acknowledgement/Returned as unprocessable claim-The claim/encounter has been rejected and has not been entered into the adjudication system.
Claim. (A3 - 275)"
What does that error mean? How do I fix it. Nothing has changed on the claims from previous weekly claims.
