Not Allowing Clients to Change the Name on The Demographics Forms

Please make it possible to lock the names on the Demographics Forms. I see children and parents. Too often parents get confused and change the names on the Demographics Forms and it causes a lot of problems--with scheduling, billing, document competition, etc. For example, I now erroneously have the same name for both child and parent or vice versa. 

I stopped sending the forms to parents to complete, but I need the other information from them. And I don't have time to complete all of that information for every parent and child. 

Having the option to "lock" the names on the forms will fix this problem!
