I'm questioning the accuracy of reminders

I noticed that I have 8 reminders, some of which are over a month old, and some of which are current. I find that I do not have Reminders for some current sessions for which I haven't done notes (and would therefore expect Reminders). Can I rely on this system to make sure I don't forget any notes? What am I missing since currently it does not appear reliable? Thanx Mark G LMFT
