Duplicate clients

I saw a woman with her husband a year ago. She has since divorced him and wanted to come to couples counseling with her boyfriend. The new couple filled out all the paperwork, but I will not be seeing them due to a "conflict of interest". But my reminders ask is this a duplicate client. Should an individual previously seen with a different partner for couples counseling considered a duplicate?

  Hi, I saw a mom for a few sessions of therapy. Now she wants me to see her child. When I add the child to the client portal, and add the mom's e-mail address, I get an error message stating that this person is already in the system. How do I enter both a mom and a child in the system (with the mom's e-mail address as a contact for the child)? Please assist.
