How can I embed an intake form on our website?
We need to have a way for people to send in their intake forms to us by filling them out online. Once we review the information, then we can give them the ability to schedule an appointment.
I see how to create the form, but not clear on how to do what we want per above.

SimplePractice does not offer the functionality to impeded our SimplePractice EHR intake forms onto external third-party websites, or to the Professional Website with SimplePractice.
With that said, you can embed your Client Portal URL as a call-to-action button on your external website. Some practices like to do so and include copy that directs clients towards logging in and completing intake paperwork.
