Help! Simple Practice Good Faith Estimate Form

I need help with how to populate the items, especially "Dates of Services"? Does this mean the scheduled dates, such as next week? Or the estimated dates of services for 12 months (which would be ridiculous, but the item is not clear on what it is asking). Also, when I did a trial run on a fake client entry, none of the "automatically inserted" items were filled in. Sigh. Please help. Like so many of the other helpful videos that have been done, a video on this would really help the most. When I reached out, most of my colleagues did not know of this law. I just encountered it accidentally last week. Yikes
