Not sure how a Fee Adjustment happened for a client

I was reviewing my billing for a client who has an outstanding balance. I noticed a "Fee Adjustment" for three sessions that I don't recall creating. I don't know what to do next and need support in figuring out how this happened b/c currently it looks like the client has a "credit" and client does not.
