Consent forms for adolescents and parents

I don't understand what happens with consent forms for adolescents.  With an older adolescent I feel that they should sign as well as their parent even if they are under 18.  There is space to input the child's email as well as the parent but I am unclear who will get the forms and if both will be able to sign.  Can anyone explain who the forms go to?  And if it's just the parent is there a way to also send to the teenager?  Thanks
