deleted/not active template

The assessments under "new" now says "deleted/not active template". I am not an admin on my account and do not have access to add forms... What can I do?

  Hi Michelle,

  Only the Account Owner and Clinicians with Administrative Access have the ability to add and edit forms. I recommend contacting them about this.
