Claims and superbills can support multiple diagnosis codes for a client. This can be useful if you’ll need to report a client’s multiple diagnoses to their insurance payer.

Enabling multiple diagnosis codes in your account
Before claims and superbills can reflect multiple diagnosis codes, you’ll need to enable them in your account. To do this:
- Navigate to Settings > Billing & Services > Insurance
- Scroll down to Claims and Superbill Settings
- Check the box for Include multiple Diagnosis Codes
- Click Save
Assigning multiple diagnoses to a client
Once multiple diagnosis codes are enabled, you can add additional codes to a client’s existing diagnosis and treatment plan, or create a new one.
To create a new diagnosis and treatment plan with multiple codes:
- Navigate to the client's Overview page
- Click New > Diagnosis and treatment plan
- Select the appropriate diagnosis code from the dropdown menu
- Click the + sign next to the diagnosis code you selected
- Select another diagnosis code from the dropdown menu
- After all relevant diagnosis codes have been added, click Save

To add additional codes to an existing diagnosis and treatment plan:
- Navigate to the client's Overview page
- Click the Diagnosis and treatment plan you’d like to edit
- Click the pencil icon
- Click the + sign next to the existing diagnosis code
- Select another diagnosis code from the dropdown menu
- Click Update
Important: If the plan is locked, you can unlock it to make your changes. For more information on unlocking diagnosis and treatment plans, see Signing a diagnosis and treatment plan.
Viewing multiple diagnoses on claims and superbills
After you’ve entered multiple diagnosis codes in a client’s file, these codes will appear on new claims and superbills.
Note: Assigning multiple diagnosis codes to a client won’t change any of their existing claims or superbills. If you’d like the codes to appear on a claim or superbill that was created before the diagnoses were assigned, you can delete the document and recreate it after assigning the diagnoses to the client. On a claim, the multiple diagnoses will appear in the following locations:
- Box 21, under Diagnosis or nature of illness or injury
- Box 24e, under Diagnosis pointer
On a superbill, multiple diagnosis codes will be listed with their full description and given a corresponding number. These will be listed in the Diagnosis Code and DX sections of the superbill:
Diagnosis codes will only appear for the billed dates of service if the appointment(s) were held after the date and time of the diagnosis and treatment plan. If the diagnosis codes aren’t appearing in a claim or superbill, you can edit the plan to reflect a date and time prior to the appointment(s) you’d like to bill, then delete and recreate the document.
